Monthly Class Pass CANCELLATION REQUEST

To cancel your Monthly Class Pass, fill out & submit this form by the 16th of the preceding month. For example, if you'd like for the Monthly Class Pass to end at the end of April, fill out & submit the form by 11:59 pm on April 16th to ensure that the Pass does not auto-renew on May 1st.

All cancellation requests received after the 16th day of the month will be processed following the auto-renew for the subsequent month of the request. For example, submitting the cancellation form on September 21st will cancel the Monthly Class Pass as of the final day of October (and it will not auto-renew in November).

I would like to cancel the Monthly Class Pass. I understand that if I want to restart my Class Pass within the next two months, my first month will not be prorated.

I understand that this request is good for one month and I will need to submit a new Skip Request for the following month(s) unless I would like my auto-renew to resume.
